This case highlights the importance of conducting a comprehensive assessment in older adults presenting with persistent fatigue. Although fatigue has many potential causes, the patient’s elevated hemoglobin A1C, medication nonadherence, nocturia, blurred vision, bilateral foot pain, and uncontrolled hypertension strongly support poorly controlled type 2 diabetes mellitus as the primary diagnosis. Early recognition, evidence-based treatment, patient education, and consistent follow-up are essential to improve glycemic control, reduce symptoms, prevent diabetes-related complications, and enhance the patient’s overall quality of life.
