AxessPointe Community Health Center: An Overview
AxessPointe Community Health Center (API) is a not-for-profit Federally Qualified Health Center located at 1400 S. Arlington St., Suite 38, Akron, OH 44306. Established in 1994, AxessPointe is dedicated to public health and is represented by the Ohio Association of Community Health Centers (OACHC). API operates as a patient-centered medical home (PCMH) accredited through the National Committee for Quality Assurance (NCQA). It primarily serves the uninsured and underserved populations across Summit and Portage counties, offering medical, dental, behavioral health, and on-site pharmacy services. Additionally, API collaborates with Portage Path Behavioral Health to provide medical services for Portage Path patients. The center also conducts various community outreach initiatives, including insurance enrollment assistance, a food pantry, and personal hygiene support, facilitated by a team of Community Health Workers (CHW) (API, 2021).
Roles and Responsibilities of Health Information Management Professionals
At AxessPointe, health information management (HIM) professionals play integral roles across departments. Despite a relatively small HIM team, responsibilities encompass various areas within the organization’s growing structure. The IT Department, comprising only two employees, manages the technological infrastructure to keep AxessPointe connected and operational. They ensure the security and functionality of electronic health records (EHRs) and address communication system errors promptly (Burke & Weill, 2019).
The Billing department oversees medical records, billing data management, and documentation accuracy. It comprises a Billing Manager, Billing Specialist, and Medical Records Technicians responsible for managing and organizing patient records and ensuring billing accuracy (Jones, 2015).
Two Registered Nurses (RNs) lead the Quality Department, managing data related to quality improvement initiatives, incentive programs, and EHR training for clinical staff (Knox et al., 2015).
Challenges of Health Information Management Professionals
HIM professionals at AxessPointe face challenges related to evaluating organizational risks and managing data effectively. Limited staffing and outdated technology pose significant concerns, impacting communication systems and overall efficiency. Addressing these challenges is crucial to maintaining patient safety and optimizing organizational performance (Ammenwerth et al., 2003).
Future challenges revolve around effective data management strategies to leverage technological advancements for improved patient outcomes (Dash et al., 2019).
Inpatient and Outpatient Facility Analysis
AxessPointe primarily operates as an outpatient facility, offering a wide range of primary care, dental, and behavioral health services. The shift towards outpatient care reflects industry trends aimed at reducing inpatient stays, enhancing patient convenience, and optimizing healthcare delivery (Abrams et al., 2018).
Technological Solution Implementation
To address technological challenges, AxessPointe can consider implementing integrated phone systems and transitioning to cloud-based servers. These solutions can enhance communication efficiency, data accessibility, and system reliability, ultimately improving patient care and organizational performance (Demillo, 2019; Kamani, 2019).
Organizational Structure of a Health Care Facility
AxessPointe operates under a structured organizational hierarchy, with a Board of Directors overseeing business decisions and senior leadership managing daily operations. Key roles include the CEO, CFO, CMO, and Directors overseeing various departments and functions (API, 2021).
Clinical Services of a Health Care Facility
AxessPointe offers comprehensive medical, dental, and behavioral health services aimed at addressing the needs of the community. The center’s focus on prevention and holistic care aligns with its mission of providing accessible, quality healthcare services (API, 2021).
Conclusion
Despite its longstanding presence, AxessPointe continues to evolve to meet the changing needs of its community. By aligning with regulatory standards and embracing technological advancements, the center remains committed to delivering high-quality, family-oriented primary care services across Northeast Ohio.
