Applying Quality Improvement to Nursing Practice
Describe one patient care scenario where a safety or quality issue is apparent. This can be something you have experienced in your current practice or one you develop for this discussion. Explain the details surrounding the scenario and the impact of the problem.
In the Post-Anesthesia Care Unit (PACU), maintaining patient safety and quality of care is a constant priority. However, unexpected clinical circumstances often reveal gaps in existing protocols. During the COVID-19 pandemic, the Intensive Care Unit (ICU) became overwhelmed with critically ill patients. To manage this overflow, a neurology patient who remained clinically unstable was transferred to the PACU. This decision introduced a significant safety concern, as the PACU is not designed or equipped to provide long-term intensive care for patients requiring continuous monitoring and complex interventions.
Although PACU nurses possess strong skills in emergency stabilization, the workflow, documentation procedures, and available monitoring equipment differ substantially from ICU standards. The situation was further complicated by staff shortages, limiting the capacity to provide consistent and safe care. As a result, patient safety was compromised, and healthcare staff experienced elevated stress levels while operating outside their normal scope of practice.
