Care coordination is the strategy that supports someone with a medical condition or a disorder in gaining access to a wide range of services and treatments that allow them to improve and overcome the disease. It entails connections between several health professionals and nursing staff, as well as the patient, their caregivers, family, and other key individuals. The purpose is to help people with mental illnesses enhance their health and day-to-day functioning. It is incredibly useful for people with chronic conditions in long-term healthcare institutions who have complicated emotional health needs. Since they are associated with several institutions and people who are attempting to manage their care requirements, these patients are especially vulnerable.
