To assess the standards of care provided to patient, certain standards are identified. These standards are basically six domains of efficiency, safety, patient-centeredness, effectiveness, timeline and accessibility. These standards have accelerated change in initiatives and outcomes to get their task completed effectively. According to the Agency for Healthcare Research and Quality (AHRQ), patient experience includes multiple aspects of healthcare service delivery that are well received by the patients at the time of service reception (HALLAÇ & KAYA, 2022).  It includes a well maintained schedule for checkup, easily available and accessed information and the ability to communicate effectively with healthcare staff and respective service providers. Kotter’s 8 step model and Lewin’s 3 stage model of change were introduced as change management assessment models (Hoeft et al., 2019). Kotter’s 8 step model creates time management awareness, assign strong leadership, creates an outcome based ideology, help maintain communication, overcome problems, offer solutions a results and maintain efficiency in the very practice of patient and their family member communication. The three steps of Lewin’s 3 stage model of change are unfreeze, change and refreeze to practice change management effectively. These two models help determine the quality of care obtained by patients through healthcare institute.

The Rationale for Coordinated Care Plans

In the coordinated care management plans, there are two rationales observed by healthcare service providers in maintaining effective communication with patients and their family members as well as effective service provision. These rationale include professional autonomy and loyalty and the regard of patient’s family members regarding the limitations of service provision staff. It’s one of the best examples is considered in Norwegian healthcare staff as their foundational ideology is framed on liberal welfare model followed under state supervision. The purpose of this practice is to allocate funds to healthcare staff and provides them an access to services which maintain transparency for decision making. The strategy of purchaser-provider service (PPS) ensures the allocation of role attribution to respective individuals within work environment (Menear et al., 2020). This increases the job responsibilities of care providers to attribute services fairly. This fair distribution has to decide between individual benefit and collective good. In cases when the patient needs and expectations are exclusive and situation bound the social and resultant impact of these behavioral obligations increases manifold. This changes the overall perception associated to the healthcare sector respective care providers are associated to. The moral values associated to these practices derive moral intentions the people withhold collectively including human life, welfare, freedom, dignity, autonomy and justice. The patient and their family members who trust to associate themself with certain healthcare institute hand over their health concerns and expected outcomes along with the expectation of best possible outcome or communication and understanding approach to the least. This attributes a magnitude of pressure to make decisions on healthcare providers. After the successful communication with the family members of the patients a harmonious continuum is maintained. The expectation of patients and their families to spare Home Health Care (HHC) which creates a compound pressure is distributed among another component of stakeholders making the successful treatment of patient a group effort (Menear et al., 2022).
