Care coordination is the strategy that supports someone with a medical condition or a disorder in gaining access to a wide range of services and treatments that allow them to improve and overcome the disease. It entails connections between several health professionals and nursing staff, as well as the patient, their caregivers, family, and other key individuals. The purpose is to help people with mental illnesses enhance their health and day-to-day functioning. It is incredibly useful for people with chronic conditions in long-term healthcare institutions who have complicated emotional health needs. Since they are associated with several institutions and people who are attempting to manage their care requirements, these patients are especially vulnerable.

Along with the clinical therapy, the patient may need care coordination. The assistance of various experts from other domains, a healthcare team, and help with self-management.  A Care Coordinator is somebody in charge of organizing, guiding, and implementing psychiatric treatment, therapy, and assistance that is personalized to each patient’s needs. The Coordinator may be employed by a specialized mental health department, a community management organization, or a primary healthcare professional. Among mental health conditions, depression has become very common. During the pandemic, cases have drastically increased all over the globe. NURS FPX 4050 Assessment 1: Preliminary Care Coordination Plan. The lockdown and shutdown of educational institutes, offices, restaurants, parks, and traveling affected the mental health of various people, but especially the healthcare workers, as they were the frontline warriors in these stressful times. Therefore, a coordination plan is important that will consider an individual’s personal, psychological, and behavioral needs as well as aid in locating resources available for an effective and robust continuum of care.

Health Concerns and Best Practices for Health Improvement

Depression is very common in the United States, affecting more than 16 million Americans (Depression, 2022). Nurses and mental health workers routinely help patients of all races and backgrounds, all with their own set of principles and behavioral expectations. Considerations about the economy, tradition, ethnicity, color, and social standing abound in this intercultural care facility strategy. Cultures provide a cultural value system that explains the cultural norms of individuals having mental and emotional health issues, immigrants, and refugees. They are continually worried or depressed about the country’s growing wealth disparities, that has a detrimental impact on their overall healthy eating and exercise and psychological health. Various aspects pour in and lead to depression. An interdisciplinary approach that is based on community-based evidence can prove to be very efficient as it can target In the United States, financial disparities have been considered the main cause of depression and anxiety among citizens. According to various reports and studies, a higher debt ratio is seen to cause these financial disparities among people (Ford et al., 2019). According to the American Psychological Association’s 2018 Distress in USA poll, the ratio of citizens who reported feeling anxious regarding finances at least in some ways in the preceding month increased (72 percent). Indeed, despite general financial conditions, expenditures have consistently remained the strongest stressor since a nationwide 2007 survey was conducted.NURS FPX 4050 Assessment 1: Preliminary Care Coordination Plan.  The debt of people was mostly characterized as personal debts such as debit cards and student loans, which are also reported to cause depression. It was reported to be a major cause of anxiety and stress for over half of the respondents questioned in 2018. That makes up about 42% of all the people that were included in the survey (American Psychological Association, 2018). Similarly, economic distress was also the main component of depression and anxiety among people all over the world during the pandemic. The pandemic has plunged the world into health and economic disparities, increasing the number of cases of depression and anxiety (Wanberg et al., 2020). Along with the community-based intervention, Nurses must also set targets to better comprehend patients’ bitter and miserable emotions in order to provide a stronger impact on their situation. To provide the best care, they must practice patience, compassion, and confess their flaws.

Goals to Address Healthcare Problems

The approach that I will be targeting for this specific scenario is that nursing staff, according to research findings, must use alternative solutions to deeply recognize standards because they face numerous challenges while proceeding to assist individuals with long-term mental disorders, such as confusion about medicine use, a lack of understanding of psychodynamic and therapy strategies, and a longer primary care time frame. Initially, individuals experiencing depression will be given assessments. A series of psychological well-being and relationship measures, economic scales, dietary and physical health queries, and legal concerns make up the evaluation. The examination also includes a comprehensive scanning tool that allows the nurses and physiotherapists to acquire a better understanding of co-occurring issues. This will help in targeting the stressors that are causing depression which will further aid an inter-disciplinary collaboration. Once the stresses are recognized, the individuals along with the therapy and medicinal regime will have to follow a procedure and the nursing staff will need to keep track of these procedures for effective treatment.

Community Resources for a Safe and Effective Continuum of Care

When an individual has been suffering from depression and anxiety for a very long time, then the effective management and treatment of these individuals cannot be done by nursing staff. The community resources, in this case, will be able to provide safe and effective care. The nurses, having their expertise in the domain of psychiatry, will work with patients in a community-based setting, assisting them to pursue the best treatment options.

One of the community resources that has been reported to be effective for people with depression is a support group. Most of the time, depression leaves one feeling lonely. Meeting with people who understand the pain one is going through can be very beneficial. This is referred to as interaction with people or peers. A support group helps people suffering from mental disorders or depression to both give and receive support. At gatherings, there is more than just listening and chatting. Many organizations host social functions and offer unique services to people aimed at enhancing their moods and emotional wellbeing. It can be intimidating to attend this session with people in groups for the first time, but people can be very friendly because they can understand each other pain and feelings that they go through (NHS Choices, 2022).

“Active Minds” is another community resource. It has over 450 locations across the United States and creates awareness and interactive media for youth dealing with mental health issues. Through coaching, research, participation, and a focus on young adults aged 14–25, Active Minds is widening the conversation on mental health and creating a lasting effect on the way mental disorders are talked about, addressed, and recognized in the United States. This will assist individuals with mental and emotional health understanding, as well as training and understanding on how to manage feelings and comprehend their psychological and emotional needs. This community resource has also been quite popular among college students. During the pandemic, they were actively communicating and providing aid at college counseling centers to help students effectively manage their psychoeducation and mental health. Because of the social distancing and lockdown, they have been assisting students via remote means, mostly through the college’s website. (Seidel et al., 2020).

Conclusion

Nurses can help patients with mental illnesses and depression by managing and treating them. Interdisciplinary collaboration can aid in pinpointing the roots of depression and anxiety by identifying the stresses that cause depression. Nurses working along with patients’ therapists can assist patients with successful self-care. Patients who have been depressed for a long time may require community services and active psychotherapy to assist them cope with their circumstances effectively.
